Every operating surface produces signals: a new registration, a broken link, a forum prompt, a chat burst, a search error, a headline shift, a strange referral, or a page that suddenly starts receiving attention.
The problem is not scarcity of signal. The problem is unranked signal. When every observation feels equally important, the operator starts treating motion as meaning.
Triage Questions
Urgency: does this require action before the next routine pass?
Evidence: is the signal confirmed, repeated, reported by a trusted source, or still only a trace?
Reversibility: can action be undone if the read is wrong?
Impact: does the signal affect safety, trust, access, visibility, continuity, or public understanding?
